ALL COD must be submitted in English! （excludes passport copy）
Due to different requirements between hospitals, and to avoid wasting your time & money, we only require partial documents to be sent with the AF. But our LEO has the right to require additional documents in order to apply for your clerkship, and you shall submit the required documents within two weeks or by the deadline the LEO set up. Regulations for COD documents to be sent with the AF
-Submit Motivation letter for 'clerkship'
in the Motivation letter for department 1 box
-passport should be valid for at least 7 months beyond your desired exchange start time
-We only accept ID Photos in the COD
-Proof of enrollment, CV, recommendation letter, student record, immunization record shall be submitted in EnglishRegulations for LC specific COD (Does not have to be sent with the AF)
-For LC Application form, HepB Antibodies count, MRSA Test, Tuberculosis test and X-ray further instructions will be sent by our LEOs after assigned to LC
-Some hospitals might require X-rays, specific vaccination, and specific tests, please follow our LEO's instruction in order to complete the application process
-All required documents are important and critical for the application processSpecial Documents
-Health and Immunization Form: For COVID-19 vaccination and PCR test negative reports
-Other requirements that does not fit the discriptions above
